Ten years ago, the RO-ILS: Radiation Oncology Incident Learning System® was launched as a major initiative of the American Society for Radiation Oncology and American Association of Physicists in Medicine to address safety concerns. Over the past decade, approximately one-third of US radiation oncology facilities have enrolled and over 34,000 safety events were reported. RO-ILS is unique as it is the only specialty specific incident learning system which is completely free to users, which emphasizes ASTRO, AAPM and supporters commitment to safety in the field. RO-ILS facilities contribute data to a standardized national database managed by a patient safety organization. Individual facilities can identify error-prone processes within their specific practice and then implement, and assess the effectiveness of corrective actions. A group of ASTRO and AAPM selected safety experts analyze the database and develop trend and case-based educational materials to the radiation oncology community. To date, more than 50 educational resources have been released to the public. There are many challenges with running a successful, voluntary national program and many lessons have been learned along the way. Multiple components of the RO-ILS program have evolved and improved since the first rollout in an ongoing process to maximize learning potential and improve data quality. This session will briefly explain the history and current status of the program, followed by the latest RO-ILS trends and findings. After discussion about the programs impact, the panel will discuss the future of incident learning and how to advance radiation oncology safety in the next decade.
