1055 - Prostate Cancer Survivors Long-Term Supportive Care Needs: Results from a Population-Based Cohort

Purpose/Objective(s): Previous research has shown the importance of identifying supportive care needs in cancer survivors. However, long-term supportive care needs are understudied in prostate cancer survivors. Further, whether racial disparities exist in specific unmet needs in cancer survivors is also unknown. This study addresses these knowledge gaps. Materials/

Methods: In collaboration with the state cancer registry, the North Carolina Prostate Cancer Comparative Effectiveness & Survivorship Study (NC ProCESS) enrolled patients throughout NC with newly-diagnosed prostate cancer from 2011-2013, and followed the cohort prospectively with surveys and medical record abstractions. At the 7–10 year follow-up time points, the validated Cancer Survivors’ Unmet Needs (CaSUN) survey was administered to assess unmet needs in long-term survivors. The 645 Black and White participants who completed CaSUN survey formed the population for this study.
Results: Out of 35 CaSUN need items, 28 (69%) had a statistically significant difference between Black and White prostate cancer survivors, with a higher percentage always with Black survivors. Representative items are summarized in Table. Numerically, the items with the largest differences between Black and White survivors were “I need help finding financial support” (difference 10.8%, P <.001), “I need a case manager” (9.6%, P<.001), and “I need help in supporting my partner and/or family” (9.3%, P<.001). A total of 37.8% Black and 26.9% White survivors reported at least one unmet need. On multivariable analysis, having at least one unmet need was associated with Black race (OR 1.73 vs White, p<.03) and younger age (OR 0.96 per year, P<.02), but was not associated with education, household income, employment status, or cancer risk group at diagnosis.
Conclusion: 37.8% Black and 26.9% White long-term prostate cancer survivors have at least one unmet need at 7-10 years after diagnosis. Racial disparities exist. These results help guide efforts to improve survivorship care.
